Georgia requires a patient to have a qualifying condition and meet the applicable program requirements.
A fully licensed physician whose principal practice location is in Georgia must certify the patient. Physicians are not required to certify a patient. Certification is based on the physician's medical judgment.
The physician enters the required patient information and documentation into the Georgia Medical Cannabis Patient Registry.
The Georgia Department of Public Health verifies the information and may request additional documentation if necessary.
If the information is verified, DPH emails the patient instructions for paying for the card. The state card fee is $30 per card. Paying through DPH's online payment portal adds a $3.75 service fee. Patients who cannot use the portal or do not have email may instead mail a $30 money order to DPH's Medical Cannabis Program. Payment must be submitted within 60 days. Physician or provider charges are separate from these state fees and may vary.
The Medical Cannabis card is currently valid for five years, and the expiration date is printed on the front of the card. Separately, your physician must complete an Annual Medical Cannabis Certification, so a five-year card does not remove the yearly physician certification requirement. When the card itself expires, DPH says to consult your physician to renew it.
Exact requirements and procedures can change. Confirm them with official Georgia sources and a qualified healthcare professional.

Georgia currently lists the following qualifying conditions for the Medical Cannabis Patient Registry.
Some qualifying conditions have specific age, diagnosis, severity, or other legal requirements. A physician determines whether a patient's medical condition meets Georgia's certification requirements.
For most Georgia residents, a current Georgia driver's license or Georgia state-issued ID showing their current address is the main document to have ready.
A current Georgia driver's license or Georgia state-issued ID showing your current Georgia address generally satisfies both the identification and Georgia residency documentation requirements.
If your identification does not show your current Georgia address, additional documentation showing your full name and current street address may be required.

Check your card's expiration date and consult your certifying physician about renewal.
DPH currently states that Medical Cannabis cards are valid for five years. An older, unexpired Low THC Oil Registry Card remains valid until its printed expiration date; the program's name change does not require you to replace an unexpired card.
DPH directs patients to consult their physician when the card expires. The steps below combine that renewal instruction with DPH's current registry, payment and card-delivery guidance. Your physician or DPH can confirm the requirements for your case.
DPH lists a $30 fee per card, plus $3.75 when using its online payment portal ($33.75 total online). If you cannot use the portal or do not have access to email, DPH accepts a $30 money order mailed to:
Georgia Department of Public HealthPhysician or provider charges are separate and may vary. UPS delivery requires someone to sign for the package. If you do not have a permanent or secure home address, or use a P.O. Box, DPH sends the card to the health department nearest your residence and contacts you when it is ready. Bring valid identification for pickup. See the existing Timing, Card Status & Delivery guidance for card tracking and missed-delivery information.
If payment is not submitted within 60 days, DPH says the card is cancelled and you must contact your physician to reinstate it. This is a payment rule, not permission to use an expired card. If your card has already expired, contact your physician or DPH for instructions specific to your situation.
